Eligibility
- All regular employees must be enrolled under the program.
- For clients aged 18 to 65 years old.
- Dependent of Married Employee:
- Legal Spouse - up to 65 years old
- Children - from 14 days to 21 years old

- List of employees with dates of birth, gender and civil status
